The signup is an EssentialElements Form Builder with one email field inline; it only stores submissions until you add your mailing list action. It is all core Breakdance on advanced grid layouts, so the band stacks at tablet portrait and the link columns go to two on phones; keep the column headings short so they stay on one line.
